1. Guardian’s commitment(16~18 years old)
Applicants who are from 16 years old to 18 years old need provide guardian’s commitment. Please fulfill the form and sign it.

*3. Passport Copy
A clear copy of your valid passport with name, photo, birth date, place of birth, date of issue, and date of expiry clearly visible.
Visa page is required for applicants who are currently living outside their country of residence.*4. Personal Statement
An essay statement written in English or Chinese on one page of A4 size paper. Introduce why you are applying, describe your interest and enthusiasm for your chosen major, and how your previous studies or experiences relate to it. Describe what makes you suitable, such as relevant skills, experience, or achievements gained from previous education, work, and other activities.

No need to provide if you are applying for 1+3 program with foundation year.
*6. High school graduate certificate
Please provide High School graduation certificate.
Copies of the original certificate with the school’s official stamp and/or the signature of the principal. If the originals are not in Chinese or English, they must be translated and notarized.*7. High school academic transcript
Please provide High School transcripts of all grades.
